COVID-19 Vaccines & Boosters


To receive a vaccination or booster, schedule an appointment below. All patients will be required to fill out a COVID-19 vaccination consent form prior to vaccination. You can print and fill out this form before you arrive or a form will be provided to you at the start of your appointment. Scheduling an appointment is recommended, although walk-in appointments are available on a limited basis.

Schedule an Updated Bivalent COVID-19 Booster Vaccine

The updated bivalent Moderna booster vaccine is a single dose to be given to those 18 years of age and older 2 months following the completion of their primary series or latest COVID-19 vaccination.

The updated bivalent Pfizer booster vaccine is a single dose to be given to those 12 years of age and older 2 months following the completion of their primary series or latest COVID-19 vaccination.

Schedule a Moderna or Pfizer Vaccine

Moderna You must be at least 18 years of age to receive the Moderna vaccine and to schedule an appointment. The Moderna vaccine requires two doses to be given 4-6 weeks apart, initially. Pfizer You must be at least 12 years of age to receive the Pfizer vaccine and to schedule an appointment. The Pfizer vaccine requires two doses to be given 3-6 weeks apart, initially.

COVID-19 Vaccination for Children 5 through 11 Years Old

Children 5 years and older can get the Pfizer-BioNTech COVID-19 Vaccine by appointment only at most Cub Pharmacy locations. The Pfizer vaccine requires two doses to be given 3 weeks apart, followed by a booster dose at 5 months or more after the second dose.

COVID-19 Antiviral Treatments 

Paxlovid Available at Cub Pharmacy

Paxlovid is authorized to treat COVID-19 in adults 12 years of age and older following a positive COVID-19 test. A prescription for Paxlovid is required from your medical provider to receive the medication. There is no out-of-pocket cost for patients.

All Cub Pharmacy locations have limited quantities of the COVID-19 antiviral, Paxlovid. If you have been prescribed Paxlovid, please call ahead to speak with the pharmacist to coordinate the pickup of your medication.
