COVID-19 Vaccines

 

To receive a vaccination or booster, schedule an appointment below. All patients will be required to fill out a COVID-19 vaccination consent form prior to their vaccination (this form will be provided to you at the start of your appointment). Scheduling an appointment is highly recommended as walk-in appointments are limited at this time.

 


Is it time for your COVID-19 Booster?

 

Here’s who is eligible to receive a second booster dose of an mRNA COVID-19 vaccine (Pfizer or Moderna):

• People 12 and older who are moderately or severely immunocompromised and received their last booster dose at least 4 months ago.

• People 50 and older who received their booster dose at least 4 months ago.

• People ages 18-49 years who received Janssen (Johnson & Johnson) COVID-19 vaccine as their primary series dose and booster dose at least 4 months ago.

    Schedule a Moderna or Pfizer Vaccine or Booster

     

    Please use the same links whether you’re seeking a first, second or booster dose of the Pfizer or Moderna vaccines.


    Moderna
    You must be at least 18 years of age to receive the Moderna vaccine and to schedule an appointment. The Moderna vaccine requires two doses to be given 4-6 weeks apart, initially.

     

    Pfizer
    You must be at least 12 years of age to receive the Pfizer vaccine and to schedule an appointment. The Pfizer vaccine requires two doses to be given 3-6 weeks apart, initially.

    Schedule a Johnson & Johnson Vaccine or Booster

     

    You must be at least 18 years of age to receive the Johnson and Johnson vaccine and to schedule an appointment. The Johnson and Johnson vaccine is given as a single dose.


COVID-19 Vaccination for Children 5 through 11 Years Old



Children 5 years and older can get the Pfizer-BioNTech COVID-19 Vaccine by appointment only at most Cub Pharmacy locations. The Pfizer vaccine requires two doses to be given 3 weeks apart, followed by a booster dose at 5 months or more after the second dose.

COVID-19 Antiviral Paxlovid Available 


Paxlovid is authorized to treat COVID-19 in adults 12 years of age and older following a positive COVID-19 test. For information about Paxlovid please see: Antiviral Fact Sheet for Patients. A prescription for Paxlovid from your medical provider is required to receive the medication. There is no out-of-pocket cost for patients. 

All locations will have limited quantities of the COVID-19 antiviral, Paxlovid. If you have been prescribed Paxlovid, please call ahead to speak with the pharmacy staff to coordinate the pickup of your medication.
